In one of the most significant moves for women’s health in decades, the Food & Drug Administration (FDA) has announced the removal of the so-called “black box” warning from many hormone replacement therapy (HRT) products used to treat menopausal symptoms. This decision reflects decades of evolving medical understanding and opens up new possibilities for individualized care. The FDA & HHS held a press conference and press release on November 10th, 2025 where they discussed a number of science-backed items related to women’s health in general, and hormone therapy specifically.
Key Takeaways from the FDA’s Decision
1. Outdated data: The original black box warning was rooted in data from more than 20 years ago — notably the Women’s Health Initiative (WHI) — which was based on older hormone formulations and study participants well past the typical onset of menopause.
2. Updated understanding: Since then, research has advanced; newer evidence supports that factors such as the type of hormone, delivery method (e.g., patches, gels, bioidentical vs synthetic), and timing of initiation (especially within 10 years of menopause or under age 60) significantly influence risk/benefit ratios.
3. Patient empowerment: By removing the blanket black box warning, the FDA aims to enable more nuanced, personalized conversations between patients and providers about whether HRT (including bioidentical hormone therapy) might be appropriate. It’s not a “one-size-fits-all” approach. HHS
New Data on Hormone Replacement Therapy
Current FDA Commissioner Marty Makary, M.D., iterated new information that has been overlooked since the initial black box warning.
“For too long, issues of women’s health have been underrecognized. Women and their physicians should make decisions based on data, not fear.”
From the FDA’s press release itself, he cited the following:
“Randomized studies show that women who initiate HRT within 10 years of the onset of menopause (generally before age 60) have a reduction in all-cause mortality and fractures. Women may also reduce their risk of cardiovascular diseases by as much as 50%, Alzheimer’s disease by 35%, and bone fractures by 50 to 60%.” (See link above).
Aqua Vitae and Menopause Myths
Recently, some of these very myths regarding the lack of studies on hormone treatments were discussed in an online webinar hosted by Aqua Vitae and FNP Maggie Ford, from our Greenville clinic.
In this video, Maggie discusses many myths – including the original FDA study that initially created the label, which you can watch here:
The myths covered in the video are:
- Myth: Your OBGYN or Primary Care Physician Will Be Able To Help You With Menopause
- Myth: There’s a Blood Test for Perimenopause
- Myth: I Don’t Need HRT, It’s a Natural Part of Aging
- Myth: There is No Need to Worry About Menopause Until Age 50
- Myth: HRT Causes Breast Cancer
- Myth: Testosterone is Only for Men
- Myth: I Don’t Have a Uterus, So I Don’t Need Progesterone
- Myth: All Progesterone is the Same
- Myth: HRT Causes Blood Clots
An example of the myths covered is that menopause is a natural part of aging, so women don’t need hormone replacement.
Here’s Maggie’s response:
“You have to remember that you’re gonna spend the majority of your life in post-menopause. I mean, you go into menopause at age 50. So there’s a lot of years after that, and we wanna do everything we can to protect the brain. So that’s key for estrogen. The other big one you’re gonna see is risk of fractures. So studies have shown that one year after menopause and without estrogen replacement, your risk of fractures increases drastically. So the idea behind taking estrogen also helps with bone density and preventing osteoporosis. And then the last one was cardiovascular. So I always tell people, listen, losing menopause or losing hormones in menopause is, I guess you could argue a natural part of aging, but just like if you lost your thyroid and needed hormone replacement to take the place of your thyroid functioning, it’s the same concept in menopause. We’re living longer. We’re going into menopause earlier. You need to protect your brain, your heart, and your bones.”
Symptoms of Menopause
Nurse Practitioner Ford also discusses the many symptoms that can come with perimenopause and menopause. Some are the normal ones – hot flashes, affected sleep. But others are more nuanced – like brain fog, dry skin, dry eyes, and more.

What This Means for You and Your Care at Aqua Vitae
At our clinic, we believe in evidence-based, individualized care for menopause and mid-life health. With the FDA’s updated labeling, here’s how we approach hormone therapy:
- We review each patient’s hormone levels, health history, symptom profile, risks and goals. We have certified phlebotomists in-house, and can do lab draws right in our clinic.
- We discuss the timing, formulation, differences and delivery method of hormone therapy – whether it be pellet, compounded or cream.
- We expand the conversation beyond symptom relief — looking at bone health, cardiovascular health, sleep, mood and sexual wellness.
- We monitor therapy over time, adjusting as needed and reassessing risk/benefit periodically.
What Does this Mean for Your Menopause Journey?
Bottom line: The removal of the black box warning doesn’t mean HRT is without risk — but it does mean that the outdated, fear-based caution is replaced by a more balanced, personalized approach. For many women, this opens up options and a pathway to better quality of life in mid-life and beyond.
